CLIENT AGREEMENT
THE SLEEVED DIETITIAN PRIVATE COACHING TERMS & CONDITIONS

By purchasing the Kickstarter 12-Week Program from The Sleeved Dietitian LLC , you agree to the following Terms & Conditions.
Coach and Client hereby voluntarily and willingly agree as follows:
By purchasing the Kickstarter 12-Week Program from The Sleeved Dietitian LLC , you agree to the following Terms & Conditions. You understand that you are purchasing private coaching services (hereinafter referred to as the “Program”) in exchange for the fee of $4,997, or the payment plan amount selected at checkout.
The Sleeved Dietitian LLC agrees to provide the services described within the Program Details section of these Terms. Coaching services may be delivered either by Jamie Mills or by one of her qualified, registered dietitians, approved and trained to deliver this Program (Sleeved Dietitian and service provider, hereinafter collectively referred to as "the Coach").
You acknowledge that the Coach (or assigned dietitian) may use suitable methodologies in alignment with their professional training and discretion, tailored to your individual needs.
- Program Details
The Kickstarter 12-Week Program begins on the date of your purchase, upon enrolment, you will receive:
- (3) 45-minute private coaching session via Zoom
- Weekly Check-In Form - Tracks your progress, challenges, and wins to keep you accountable.
- (12) weeks of Voxer coaching (voice notes and text support) for ongoing guidance between calls.
- Full access to the T.R.I.B.E. community during your coaching period, including exclusive resources, peer support, and motivation.
- Customized Routines & Goals - Tailored plans for nutrition, habits, and mindset to fit your lifestyle and set you up for long-term success. (please note that macro plans and customized meal plans are not included)
- Weekly Check-In Forms must be submitted by 11:59 PM each Sunday. These are required for personalised support and progress tracking. A reminder email will be sent each Saturday morning. It is the Client’s responsibility to submit their check-in on time.
- VOXER COMMUNICATION & CHECK-IN POLICY
To ensure a structured and effective coaching experience, the following guidelines apply to Voxer (text and voice note) communication and weekly check-ins throughout the 12-Week Kickstarter Program:
Weekly Check-In Form
- Every Saturday, the Client will receive a weekly check-in form to reflect on the previous week.
- The check-in form must be completed by midnight (11:59 PM) on Sunday to allow for review.
- Failure to submit the check-in form by the deadline may result in delayed or forfeited feedback for that week.
Voxer Communication Schedule
The Coach will check and respond to Voxer messages on Monday, Tuesday, and Thursday each week.
- The Client may send messages at any time; however, responses will only be provided on the designated days.
- If the Client requires support before the weekend, they must submit their Voxer message by 12:00 PM (Noon) on Thursday to ensure a timely response.
Voice Note & Message Guidelines
- The Client agrees to limit individual voice notes to a maximum of three (3) minutes per message before receiving a response.
- The Client is encouraged to be concise and specific in their messages to facilitate clear and effective support.
By signing this agreement, the Client acknowledges and agrees to adhere to these communication policies to maintain an efficient and structured coaching relationship.
Call Booking Policy:
Clients are responsible for booking all included sessions within the timeframe of their one-to-one coaching agreement. While automated reminders may be sent, they should not be relied upon.
The Sleeved Dietitian team will not personally follow up if calls are not booked. Any sessions not scheduled and completed within the allocated timeframe will be forfeited. These sessions do not roll over and are non-refundable.
To make full use of your package, we recommend booking your sessions in advance in the hub to secure availability.
- Confidentiality
- These Terms & Conditions are considered a mutual non-disclosure agreement, meaning both Client and Coach agree not to disclose, reveal, or make use of any confidential information learned by either party during discussions, coaching sessions, calls, emails, or otherwise. Such “Confidential Information” includes, but is not limited to, mindset information, coaching strategies, or other methodologies Client learns as a result of working with Coach, plans or outlines for future programs or packages, information contained in documents or any other original work created by Coach, and any and all other intellectual property (discussed below.)
- The Client and Coach agree that the obligation to keep Confidential Information private continues beyond the duration of the Program. This means Client and Coach both agree to continue to keep Confidential Information private, even after the completion of working with Coach.
- Should Client breach this provision and disclose confidential or proprietary information belonging to Coach or another participating in the Program, Client understands additional action may be taken by Coach up to and including legal action, including legal action if necessary.
- Client also understands that due to the potential for a “group” component of the Program, he/she may also obtain access to or otherwise become exposed to confidential or proprietary information belonging to other clients within the same group component of program via the community chat board that may be available to members within the membership portal. Client understands and agrees he/she will not disclose, steal, use, distribute, copy, or otherwise share or use any proprietary or confidential information belonging to another client in the group program. Client understands if he/she violates this provision, he or she may be liable to the third-party group member for infringement. Client will hold Coach harmless from any such third-party action taken against Client for such infringement or disclosure.
- Intellectual Property Rights
- Client agrees and understands that Coach has created numerous original, creative works in connection with the Program, and agrees that Coach maintains all copyrights and other intellectual property rights in all original or derivative content associated with or included in the Program, whether created prior to working with Client or specifically for Client, including but not limited to: documents, charts, emails, graphs, products, systems, processes, handouts, worksheets, tutorial videos, trade secrets, mindset strategies, weight management strategies, guides, and any other original work created by Coach. Client agrees she may be granted a limited right to use selected materials in the course for his or her own personal use but understands that the rights remain with Coach. Nothing in this Agreement shall constitute a transfer of ownership of any Intellectual Property from Coach to Client, nor grant any license to use the information, other than that which is expressly provided throughout the course of the Program.
- Client agrees and understands he/she is not to copy, repost, alter, publish, sell, assist others in selling, manipulate, distribute, or in any way exploit any of the content or intellectual property provided by Coach or obtained through working with Coach, without Coach’s express written consent. If such behavior is discovered or suspected, Coach reserves the right to immediately end your participation in the Program without refund, as well as access to any program or materials you may have purchased, without refund, and reserve the right to prosecute any actionable infringement or misuse to the full extent of the law.
- Licensee Rights: Coach’s Limited License to Client: Client understands that in purchasing the Program, she/he is gaining access to view all content and information available as part of the Program, as well as any additional information or content shared with him/her by Coach as she sees fit. Client understands this means he/she will have been granted a limited, revocable, non-transferrable license to read and use the information provided for use in his/her wellness, as instructed or allowed by Coach. As a “Licensee,”
Client understands and agrees that Client will not:
- Copy, edit, distribute, duplicate or steal any information or any Content obtained through Program without written permission by Coach;
- Post, distribute, copy, steal or otherwise use any portion of the Program or its content without written permission by Coach, and understand that any such use may constitute infringement, which may give rise to a cause of action against Client.
- Share purchased materials, information, content with others who have not purchased them.
- The Client understands that any such actions constitute a breach of these Terms & Conditions and may also violate United States federal copyright laws. The Coach reserves the right to take legal action in such cases.
- Payment
- The Client agrees to render payment via credit or debit card, ACH, or third-party financing provider (e.g. Affirm, After Pay, Klarna) and understands that the full purchase amount is due and payable at the time of enrolment. Unless otherwise agreed in writing as part of a payment plan, payment in full is required before access to the Program or any related services will be granted.
- Payment Plan:
- You have multiple options for how to pay for your coaching services which includes credit or debit cards and installment loans. For your convenience, we are making Affirm, AfterPay, and/or Klarna available to you. Affirm, AfterPay, and/or Klarna allows clients to pay for services over time in fixed amounts without deferred interest, hidden fees, or penalties. The Sleeved Dietitian, LLC is not affiliated with Affirm, AfterPay, and/or Klarna and does not receive compensation for making this option available. You must carefully consider your ability to repay before taking any loan. If you are interested in learning more, you can begin your loan application with Affirm above. It is quick and secure, and you'll get a real-time decision.
- Refund Policy
- The Client understands that all sales are final and no refunds will be issued once payment has been made. By enrolling in the Program, the Client agrees to this policy and waives any right to a refund.
- Should Coach experience an unforeseen event causing her or her team to become unavailable or otherwise unable to complete the Program, Client may be entitled to a partial refund on a case-by-case basis, based upon the portion(s) of the Program unable to be delivered by Coach. Reasonable, minor changes or modification to the Program made by Coach do not qualify as an inability to deliver services, and do not qualify Client for a refund.
- Client further agrees and understands that changing his/her mind about the Program, failing to follow through or understand the details of the Program, not experiencing the results he/she expected or desired, or experiencing any other similar situations does not entitle her to a refund.
- If the Client chooses to pay using a third-party payment processor such as Affirm, Afterpay, Klarna, or any similar financing platform, they understand and agree that The Sleeved Dietitian, LLC is not responsible for the terms of that financing arrangement. This includes, but is not limited to, payment schedules, fees, interest, approvals, or refund requests. All program purchases, including those made through a third-party payment plan, are final and non-refundable through The Sleeved Dietitian, LLC. Any questions or concerns related to payment terms, billing errors, disputes, or refund requests must be handled directly with the third-party provider.
- Indemnification
- Client agrees at all times to defend, fully indemnify and hold Coach and any affiliates, agents, team members or other party associated with Coach harmless from any causes of action, damages, losses, costs, expenses incurred as a result of Client’s use of Program, as well as any third-party claims of any kind (including attorney’s fees) arising from his/her actions as a direct or indirect result of Client’s participation in Program. Should Coach be required to defend herself in any action directly or indirectly involving Client, or an action where we decide Client’s participation or assistance would benefit Coach’s defense, Client agrees to participate and provide any evidence, documents, testimony, or other information deemed useful by Coach, free of charge.
- Voluntary Participation
- Client understands and agrees that he/she is voluntarily choosing to enroll in Program and is solely responsible for any outcomes or results. While Coach believes in the services and that Program is able to help many people, Client acknowledges and agree that The Sleeved Dietitian, LLC is not responsible nor liable to Client should Client sustain any injuries, incur harm, or encounter any negative ramifications. Client agrees that he/she is fully responsible for his/her health and well-being, including participation in Program and any results therein.
- Disclaimer
- Client agrees and understands that Coach cannot guarantee any specific results, outcomes, weight loss, or changes to Client’s current situation, and will hold Coach harmless if he or she does not experience the desired results. Client is entering into this agreement voluntarily and of his or her own free will, and readily understands that he or she may or may not experience results desired, or achieved by other clients of Coach.
- Client understands that all services provided by Coach in connection with the Program being purchased are provided on an “as is” basis, meaning it is without any guarantees, representations, or warranties, including but not limited to warranties relating to quality, non-infringement, fitness for a particular purpose, merchantability, or expectation or course of performance. Client is choosing to purchase this Program and work with Coach on a purely voluntary basis and does not hold Coach responsible should Client become dissatisfied with any portion of the Program.
- Client agrees that he/she does not have a cause of action, legal remedy, and is not entitled to a refund should he/she not achieve the results desired following completion of his or her work with Coach, as long as Coach delivers the Program as described in Paragraph 1 above, or similar substitutes, upon additional agreement by Coach and Client.
- Client understands fully that any mindset information provided by Coach is not intended to diagnose, treat, cure or prevent any type of physical or mental disease or condition. Client understands that if he/she needs specialized dietary planning to treat, cure, or prevent any type of disease or condition, Client should consult with his/her medical doctor.
- Client is solely responsible for creating and implementing his/her own physical, mental and emotional well-being, decisions, choices, actions and results arising out of or resulting from the coaching relationship and his/her coaching calls and interactions with the Coach. As such, the Client agrees that the Coach is not and will not be liable or responsible for any actions or inaction, or for any direct or indirect result of any services provided by the Coach.
- Client understands coaching is not therapy and does not substitute for therapy if needed, and does not prevent, cure, or treat any mental disorder or medical disease.
- Results Disclaimer: Coach also does not make any guarantees or assurances regarding a particular weight management or specific health outcome based on use of Program, nor is Coach responsible for Clients weight maintenance, or any increase or decrease in weight or client’s health based upon information within Program. Any information or testimonials regarding past or current clients’ participation in programs or working with Coach contained on Website or in sales material on any printed or online platform that contain specific health information are individual, and results may vary.
- Dispute Resolution
- Should a dispute arise between Coach and Client, the parties agree to attempt to resolve by good-faith negotiations and discussions. (Client agrees that failure to see results is not a basis for a “dispute” and agrees he or she does not hold Coach responsible for any specific results, or those results which have been achieved by other clients of Coach.)
- If unable to reach a resolution informally, prior to commencement of a lawsuit Client and Coach agree that all disputes will be submitted to mediation in Hartford County, Connecticut, within a ten days with written notice by either party. Client and Coach agree to participate in the mediation process in good faith and in a manner that will effectively and efficiently resolve the dispute at hand, including the exchange of any materials, documents, or information. Any settlement agreement by mediation is to be enforceable in any court of proper jurisdiction as a judgment of law or decree. Client waives the right to bring any claims through the judicial process.
- Applicable Law
- This Agreement shall be governed by and under control of the laws of Connecticut regardless of conflict of law principles, and regardless of location of Client. Client understands this and agrees that the laws of Connecticut are to be applicable here.
- Amendments
- These Terms & Conditions may only be amended or updated by The Sleeved Dietitian, LLC, and any such updates will be made available on the relevant website or platform. Continued access to the Program after updates constitutes acceptance of the new Terms.
- Severability
- In the event that any provision or clause of these Terms & Conditions conflicts with applicable law, such conflict shall not affect other provision of these Terms & Conditions which can be given effect without the conviction provision. The provisions of these Terms & Conditions are declared and agreed to be severable.
The Client may cancel their enrollment within three (3) business days of purchase by submitting written notice to the Coach via email.
If cancellation is received within this 3-day window, the Client will not be responsible for the Program fee and will receive a full refund of any payment already made.
After this period, the Program is considered non-refundable as outlined in Section 6.
These Terms & Conditions represent the entire agreement between the Client and The Sleeved Dietitian, LLC.
They supersede all prior discussions, emails, proposals, or agreements between the parties.
The Client understands that if a portion of the Program or an expectation is not explicitly stated in these Terms & Conditions, it is not included in the Program.
The Client confirms they have taken all necessary steps to seek clarification, ask questions, and understand what is included, and that they are enrolling with a full understanding of the Program as outlined.
I have read the policies of the Sleeved Dietitian LLC and understand that if I do not book sessions within the timeframe set in my program, I forfeit my right to further sessions booked outside of the timeframe. I further understand that reminders are merely courtesies and I will not rely upon reminders. I understand and agree that I am not owed refund of payment due to my failure to book sessions within the allocated timeframe and will hold harmless the Sleeved Dietitian LLC in the event I fail to book my sessions according to program policies.
